What this site collects
This website has no appointment form of its own and collects nothing from you itself. The “Request an Appointment” button opens the practice's own appointment request form — hosted by Google Forms — in a new tab. That form asks for your name, your email address, your phone number, a few scheduling details, and the state you are in. What you enter there goes to the practice; this website never receives or stores it.
Please do not include medical information in this form. We will contact you to schedule.
How we use it
We use what you send only to respond to your request and to schedule your appointment. We do not sell it, and we do not share it for marketing or advertising purposes.
How your request reaches the practice
Submitting the form sends your request to the practice so a team member can reply and arrange a time with you. If you choose to book through the practice's own Headway page, that booking tool handles your appointment details under its own privacy policy.
No advertising trackers or analytics
This site runs no advertising code, no third-party analytics, no session-recording tools and no other tracking scripts, and it sets no advertising or analytics cookies. That is why you are not asked to dismiss a cookie banner here.
Crisis support
This website and its appointment form are not monitored for emergencies. If you are in crisis or thinking about suicide, call or text 988 (Suicide & Crisis Lifeline) or call 911.
